I need help
First, stop the disgust. That only makes the problem worse. Commit to change, OK, but drop the self-loathing. If you can't - see a couselor.
If you have never gotten to goal weight, you will likely need help - IMHO, find a support group - at your or another surgeon, OA, WW, or other support, whichever works for you.

Before I had surgery, my surgeon told me to expect some bounce from goal (up to 20 pounds back****il the NUT and I could work out exactly what my needs for maintenance would be. Then, I will work on losing the bounce and settle into a calorie/protein/carb goal for maintenance. So, we agreed on a lower initial goal weight than anticipated. It's square in the middle of "normal". She said that when I get down there, I may even want to go a little lower; but it's my choice exactly where I want to stop.
Perhaps you can think of this period as your bounce. Cut the carbs, increase the solid proteins and keep your calories under 1000 until you get back to goal. Then add just a few calories (like 10) each day for a wee****il you gain. Then cut them out. Of course, your exercise level will have a little impact on how much you can consume and maintain... but for me this is EXTREMELY little impact.
Visit your Dr. and tell him/her what's going on... most of them understand that obesity is a chronic disease that requires chronic treatment... they will help you and if they don't find a Dr. that will. Get your labs done (and not just general ones, a full bariatric panel) see where you are at and if anything else maybe going on. Are you on medications/changed medications? (lots have weight gain as a side effect) ask your Dr. about alternative treatments that don't.
Build your team... good Dr, dietitian, therapist (most important IMHO), group, friends/family that are supportive. Healthy ways to work through the stress... yoga, meditation, running, something.
Purge your pantry, fridge of stuff that might have creeped in that isn't the best choice... make your house a safe zone.

None of these suggestions you've received will help unless you get to the core issues... that stress that has derailed you... figure that out and I dare say the rest will come together ;)

Congratulations on losing so much weight AND KEEPING MOST OF IT OFF. My advice would be 1) focus on the positive and the fact that you've kept the majority of your weight off (our minds are the most dangerous place); and 2) I would encourage tracking your calorie intake and exercise. If you can join a support group, think that's very helpful.
