That would be a good question for your HR department.
Most people are generally away from work for 2 - 3 weeks, using sick time and (maybe) short-term disability. I don't believe I've seen anyone needing more time than that unless there are complications that require re-hospitalization.

FMLA allows for time off from work to care for a "serious health condition". A surgery easily falls under the guises of a "serious health condition". FMLA is unpaid leave- but company policy may allow for paid leave, disability etc to be used with FMLA.
Typically you have to work for your employer for 1 year or 1250 hours to qualify for FMLA. additional, smaller employers may not be should provide FMLA coverage. Talk to your HR group though- short term disability etc may come into play and how it works in conjunction with FMLA would be up to your employer.
