A DS is not "better" than an RNY -- they are simply different with different average outcomes.  A revision to RNY would make your stomach a little smaller than it is now and give you a brief window of malabsorption.  You are already halfway to a DS.  In terms of how they work, an RNY relies primarily on restriction whereas a DS relies primarily on permanent malabsorption.

I don't know where you got the idea about eating high-fat with a DS.  In fact, eating high fat is how you end up with messy business on the back end.  You need to eat properly - a well-balanced, nutritious diet - with all procedures.

Which brings me to my rather unpleasant point -- you're not ready.  If you "never lost near the weight" and bounced "most" of the way back in just two years then you frankly never came close to making the lifestyle choices you need to.  Bariatric surgery is a reset button, a second chance, but the only thing that makes you successful in the long term is your lifestyle and your choices.  It doesn't sound like you were committed to changing and a revision isn't going to fix that.

DS'ers do need to eat high fat. We only absorb a small percentage so in order to get the proper number of fats to be healthy (not constipated and not dry skin/hair) we must eat a lot of fat. I know many who add butter to their coffee to get their fat in daily.

Interesting.  That is contrary to my surgeon's plan, everything I researched at the time of my own surgery, and everything I can find through 30 seconds of Googling.

From the first hit, from the University of Minnesota's "Diet Guidelines for Duodenal Switch":

  • Stay away from high-fat foods. You body cannot absorb fats as well as it did before surgery. High-fat foods may cause diarrhea (loose, watery stools), frequent bowel movements and increased gas. Large amounts of high-fat foods may also slow your weight loss.

Just like surgeons recommend taking just a multi vitamin and you'll be fine. Surgeons are there to cut, most of their aftercare is atrocious. My plan said to eat mashed potatoes for soft food - tried that once and was in so much gas pain, and stinky gas that I learned a valuable lesson. Many of the surgeons just give RNY advice for DS never mind the differences. Go to a DS support group/website/Facebook page, heck even DS page on OH and you'll find the real truth. Just giving out my experience and all I've read in the last 1-2 years on different groups/pages.

DSers will have major problems going low fat. The diarrhea, loose stools, and gas are from too many simple carbs, starches, sugars, etc., not fat.

Do you have any medical sources?  I quoted a diet plan specifically for DS patients that stated fat is the cause of those problems.  Here's another source:

"Reducing fat intake will usually have a beneficial effect on issues relating to diarrhea or loose stools. "

Surgeons "plans" are like as$h@les. Everyone had one and not one is like the next.

One time I had a doctor that wanted to put me on steroids for herpes simplex something. The reason? I was getting large blisters on my left butt cheek. A week later I woke up and realized that area hurt again but this time I realized my waterbed was low on water and I was laying on the heater !! At that time I realized doctors aren't always right.

I think I'll take vets with 10, 15 years of real life experience advice. I will say if you over do the fats you can get diarrhea, the trick is too find the sweet spot. Cause if you don't - constipation is a killer.

thanks for stating the same thing chili

I wouldn't use "butter in their coffee" as a sales pitch  - stick to the approved bacon mantra!

LOL. Evidently it tastes pretty good, I don't drink coffee so I'm not sure. I do enjoy the bacon though :)
