Newbie here with a question

For my 6 months I saw a behavioral therapist once a month and a nutritionalist once a month. Insurance covered this after deduct able.

For my 6 months, I saw my regular physician. My insurance company required the doctor submit notes from those 6 visits as proof that I met that requirement. 

I also found my insurance company less than helpful.  They didn't really understand what I needed to do, even though it was their rules!  My surgeon's office knew exactly what was needed to meet both their own requirements and my insurance requirements, so that is what got me through all the paperwork and everything.  My suggestion is to find a surgeon who will take you as a patient and agree to your insurance rules.  They will then guide you through the rest of the way.
