I don't see what you are claiming on this web page.

Table 5 still clearly indicates 200% of the USRDA for RNYers.  Do have super secret pre-release updated guidelines you can point us to?

Firstly, the ASMBS 2008 guidelines are a bit out of date and this is why they are being re-written. Those practices still following these guidelines are out of date and should really consider the more recent 2013 AACE/ASMBS/TOS guideline. In addition, we have to remember that these are nothing more than guidelines, not standards. It is the responsibility of the patient and bariatric program to encourage regular follow ups that include laboratory assessment so that dosages and recommendations on vitamins/minerals are done on a case-by-case basis. In any case, even if the 2008 guidelines were being followed, the guidelines actually says that 100% of DV should be met for at least 2/3 of the vitamins (see the asterisk in the original guidelines). The suggestion for GBP to take 200% DV has actually been taken out of the 2013 AACE/ASMBS/TOS paper. In terms of a multivitamin, it is suggested to take the number of tablets to meet minimal requirements. The minimal requirements for bariatric patients is not expressed in terms of DV since DV is based on the general population consuming a 200 Kcal diet....in this case minimal requirement is that the multivitamin/mineral should contain iron, folic acid and thiamin. We get into very tenous territory to dictate the ideal dosages for bariatric patients since different surgeries and individual variation and lack of research finds it difficult to dictate such dosages. The guidelines are suggestions, a starting place. Secondly, in the comment "Bariatric Fusion does contain some calcium citrate, which gastric bypass patients can absorb, but not enough to meet their daily needs", well again, erroneous....the 2013 guidelines lowered the calcium recommendations from 1500-2000 to 1200-1500 mg to meet current research. In addition, although calcium citrate still appears to be the calcium form of choice, it should not be stated that "calcium carbonate cannot be absorbed even in patients with GBP". Perhaps the person making these statements should attend the scientific meetings to be more up to date on current findings and research in the bariatric population. Also, calcium carbonate does have a higher % of elemental calcium and coupled together with calcium citrate as BF fusion contains is advantageous to keep the size of the pill manageable as well as promote compliance with taking them. And finally the last erroneous statement "Bariatric Fusion, when taken as directed on the label, doesn't come close to meeting the needs of gastric bypass patients", well, this statement is off the mark again and perhaps the person saying this should re-evaluate current guidelines and compare more accurately the BF formula.

The published 2013 guidelines still say 2 multivitamins a day.

Instead of jumping in a running your mouth it had nothing to do with taking 2 multivitamins a day. I take 4 pills a day. 4 all inclusive pills that are approved by the AMBMS.  It contains all that we need in a reformulated chewable. It also includes the calcium citrate and EVEYTHING ELSE. Kelly as usual poked her nose in an cut me off saying they are not to the standards even though I was reading the ******g bottle. So no,  She does not know everything. The bottle indicates to chew 2 in the morning and 2 in the evening. She said that is should take 8 a day to meet the outdated guidelines. She doesn't know everything. She has no business giving information.

Just because the vitamin bottle SAYS you only need 4, if taking 4 doesn't meet the 200% of the RDA that the ASMBS recommends, then it ISN'T enough to meet the requirements.  I could put Flinstones in a bottle and slap a label on it that says it meets all the needs for WLS patients,but that wouldn't make it TRUE! 

Until you can show that the alleged new guidelines say you no longer need 200% of the RDA as a RNYer, then she is correct in saying that the 4 multi's you are taking so not meet the recommendations for all of the major vitamins.  She is just doing the math, not claiming to know everything.

I still fail to see how she "has no business giving information" if all she is doing is providing the info straight from the ASMBS.

In any event, I am done with this.  You have provided NO information whatsoever to back up the claim that what she is telling people is outdated OR dangerous.

The newer guidelines are being written and will be posted around November. The guiltiness that you are quoting to people are from the ABMBS is from their latest update posted from 2008.
