Your PCP can prescribe an oral med that can take care of the fungus. You'll need a liver profile (which is a blood draw) to make sure your liver is okay before they order it. After that we generally will order labs once monthly to make sure all is well. Most people only need to take it for 3-6 months at the most. Still...I've heard that continuous nail polish, like was posted earlier, actually can take care of it as well.
I have had a nail fungus before. The prescription medicine Lamisil is very difficult on your stomach (this was pre-op)...I took it and it really didn't help. What did help was drilling down the thick part of my nail with a nail drill and applying the fungus stuff that Lora was talking about. My nails look fine now. I get pedicures every two weeks, however, I take my own pedicure set with me to the nail salon.
In addition to some of the advice here, I'll pass on what my podiatrist said - use lotramin or lamisil on your toes. Not just on the nails but rub it in daily to the rest of your feet. Fungus travels from the skin to the opportunistic nail (like after an injury or underneath polish that traps water against the nail) so if you keep the skin fungus free, you'll keep your nail fungus free! Tea Tree Oil is good, also letting your toenails breathe between pedicures and no more than 2 weeks with one polishing! Filing down the top of the nail while you apply topical treatment (either TTO or Fungicure) and have some patience while it grows out...
I know, it's a PITA. You should see my toenails - horrific!! With long-distance running, I suffer from subungual hematomas (look that one up on WebMD) so, sandal & flip flops will not be in my future this summer... if it ever stops snowing, that is!

I took my polish off one of my toes one day and found it looked thick and icky, I was horrorified. I googled it and started treating it with over the counter nail fungus repair stuff that you paint on everyday, it took a long time but the new nail grew out completely normal and has looked perfect ever since. I rarely get pedicures except in the summer when I wear sandals and this happened over winter. It did have to grow out to get better, it never hurt just looked thick and ugly. I never thought it was a big deal so I didn't call my pcp which is rare, I typically call her any time anything seems wrong.
I would go to your PCP. If he deems necessary, he could prescribe Lamisil. It gets rid of fungus of the toenails and fingernails. People are right when they say if you let it go that your nail could fall off. My ex had a fungus under his big toe, went to the PCP and got some lamisil. It did clear it up but took some time. I would go see the PCP or you may want to see a podiatrist. Which ever you would be more comfortable with. Best wishes!
