For me it took a about 3 days to be fully effective.
I would say it depends on how constipated you are.
At my worst I couldn't go for at least 5 days. Tried two suppositories and an enema. Thought I would give birth on the toilet- nothing.
On my Dr's recommendation I took the daily dose once and hour.... three times on a Friday evening and then on the 5th dose on Saturday I finally went.
Now two years out I take a tsp every morning.
I put the daily dose in my morning coffee and that seems to be effective. My worst bout of constipation happened two weeks after surgery. Eventually I continued the Miralax and took a dose of Milk of Magnesia - problem solved (stick close the bathroom). MOM isn't good for us long term but it can be handy in emergencies so says the PA at my BC.
The Miralax helps keep me regular if used daily. However, those few occasions that byproduct was hard and painful, nothing worked better for me than a glycerin suppository. Within1.5 to 2 hours, the stool was soft and easily passable.

I have been eating Kashi Protein cereal (3/4 cup=10 grams protein plus 8 grams protein from milk). Ever since I started eating the Kashi I have gone every day. Before that I was taking Miralax and still couldn't go. I feel so much better!!
