Needs must. If you HAVE to just get on with it, the chances are you will.
Don't change your plans, just make the effort you need to make! I had an 11 month old child and no help - I didn't have the time or opportunity to be a princess after my surgery and guess what? I got on with it!
You may have to make accommodations, but this is not about you - it's someone else's wedding so if you WANT to take part - you will!

I'd try and postpone the surgery until after the wedding, so that you can eat and drink and be merry. The costume will be the last thing you would worry about come wedding day because you'll likely be hungry (even if it's only head hunger) , and tired of liquid. Also you wont really have the energy for rehersals. And if you drink alcohol, you can kiss that goodbye now. What's a wedding without the bride's drunk sister? Try and get a date immediately after the wedding.
