There is no added protection or benefit to increasing your omeprazole if you already take it daily. But you could provide a little extra physical barrier by drinking milk (or carafate if you have some handy) before taking the ibuprofen. I haven't read about your current issue, but I am aware of how conscientious you are about your health and protecting your "tool" so I can imagine you must be feeling trapped between a rock and a hard place. I'm sorry to hear it. Are you planning to just take the ibuprofen for a few days short term to get the inflammation under control? If so, I would honestly recommend you up your dose to 800mg a couple of times daily to really hit it hard. That way you will have overall less time that you are exposed to the risk of ulcer with maximal results for your back. Good luck to you, hope you feel better.

I was thinking of taking 800 mg.  I do feel trapped.  I feel like I have to choose between an ulcer and horrible back pain, and that's a ridiculous choice to have to make.

I forget if you've mentioned this, but have you seen a pain management doc?  There may be nerve blocks or other things they can try beyond oral meds.

I have a nerve stimulator in my head to help prevent migraines, but nerve stimulators were actually developed for back pain.  It's a major step, but it may be worth investigating.  Again, a pain doc would be the one to talk to about it.

I would live with the back pain as opposed to the risk of damage from an ulcer. In my not so humble opinion.
Have you tried Robaxin/Methocarbamol? I get lower back pain and take tylenol with Robaxin 500mg usually a 1/2 will do as needed every 8 hours. This works well for me. It is a muscle relaxant so it may make you a bit tired, it does me at times and then others not so much.
The omperazole you should do 20mg SID for 4 weeks to be safe. If you want to be really safe get on some carafate.

No, I haven't tried Robaxin.  My orthopedic doc, who specializes in back problems, won't prescribe anything other than Vicodin.

You might prefer to live with back pain as opposed to risking an ulcer.  I would prefer to live with minor or even moderate back pain myself.  But daily back pain severe enough to wake me up several times a night, pain severe enough that I cannot bend over to get my laundry out of the dryer but have to get my partner to help me, pain severe enough that I can't bend over to get groceries out of the cart when I shop but have to ask the cashier to help me, pain severe enough that even after taking Vicodin I am sometimes in tears, pain I have lived with for more than a year?  An ulcer sounds like a walk in the park to me compared to this.

Oh my goodness, I do support you in getting to a pain management clinic and trying some of the other options other than NSAIDS. Why add pain to what you already have?

My stepdaughter had surgery a couple of months ago putting wires in her spine that she controls with a little unit, and it has been very successful for her.

Kelly no you dont want an ulcer, yes you want relief.  I saw my surgeon Tuesday....I had an accident broke my back in 4 spots I live with constant pain I understand.  Prior to surgery I could take 2 liquid Aleve capsules and I was great...love me some Aleve, now of course since surg no nsaids...hate taking Vicodin..so I asked.  Ok for short time use, occasional use, not daily long term, nor every other day long term, couple times a month on bad days, during severe pain for a few days ok.  Best to use least amount you can, but that you do aquire relief meaning not just 200 mg, take more if needed but if you dont need 800 dont take it take 600 or 400.  To me Aleve works better to each their own...did not ask this but less pills to me seems better that being said you take less Aleve to equal same Ibuprofen relief.  Good luck.  And Tramadol is an Nsaid too.
