appt at cleveland clinic today about my back
Speaking of which, I was looking through the records and noted that the doctor wrote that I reported physical therapy helped my back. I certainly don't remember saying that and I don't remember it helping, either. Has anyone else ever noticed how often doctors document things that are incorrect? I once found where one doctor had written that I was admitted to the hospital through the ER, when in fact the very same doctor had admitted me to the hospital directly; I had not gone to the ER. I have found numerous other errors in my records over the years, usually things that seem pretty small or insignificant, but still...
Anyway, I hope I'll have good news to report this afternoon!

This is one good thing about electronic records and sharing them with the patient. It takes time, but it makes for a more informed and engaged patient.
