Topic: RE: Aetna POS II Revision Before Two Years

I do not have Aetna I have Cigna. I was just going by what you wrote.

Conversion to a sleeve gastrectomy, RYGB or BPD/DS is considered medically necessary for members who have not had adequate success (defined as loss of more than 50 % of excess body weight) 2 years following the primary bariatric surgery procedure and the member has been compliant with a prescribed nutrition and exercise program following the procedure;

I don't understand what you mean by additional items. When it comes to a revision there are either complications or non-compliance.

I had a revision from band to bypass due to medical necessary. Even with an empty band it was tight and the acid reflux was horrible. I was eating tums like tic tac to get any relief. I tried to eat dense protein, but no matter how well I chewed or what type of meat it was I would vomit. When the revision was done it was found that the band was encased in scar tissue and that is what was causing my band to be tight. Had I not had the band out it would have eroded into my stomach.

So I will ask again what reason do you need a revision? What did the doctor find in your sleeve that you need to have a revision?

Doctor went through the additional items with me and believes it to be medically necessary.

Do you have Aetna? Would love to understand your background to further understand yooy interpretation of the bulletin. Thanks!

From what I am reading it appears you have to wait the 2 years and it would have to be medically necessary in order to qualify for a revision. You would also of had to of been compliant with the prescribed nutrition and exercise program following the original procedure.

What is wrong with your sleeve? Why do you want a revision?

Topic: Aetna POS II Revision Before Two Years

Hi all,

I got the sleeve August 2017. I'm looking to get revision to either bypass or DS. Ideally, I'd like to knock it out quickly. Given I can get everything taken care of for pre-op diet, etc. I'm trying to understand if I could get it before the 2 year mark? The policy is a bit confusing:

Conversion to a sleeve gastrectomy, RYGB or BPD/DS is considered medically necessary for members who have not had adequate success (defined as loss of more than 50 % of excess body weight) 2 years following the primary bariatric surgery procedure and the member has been compliant with a prescribed nutrition and exercise program following the procedure;

Does this mean I need to wait till after the two year mark to submit to make sure I don't get debite for that reason? Has anyone submitted to Aetna POS II prior to the two year mark and been approved for coverage?

Any help would be much appreciated!

Topic: RE: Have LapBand wanting to revise to Roux N y

I had my surgery Monday morning and came home on Wednesday. They sent me home with pain meds, but I didn't need any pain meds after I got home. Just be sure you walk in the hospital and after you get home. I made myself walk at least once an hour. The only complication I had was that one of my sutures got infected. I called my doctor and they put me on an anti biotic immediately and it cleared up with no problem. I volunteer at my church every Monday counting the offering. I was able to go do it the next Monday with my husband driving me. I am retired but if I had been still working I probably could have gone back after the second week if I had too. You will be tired from the surgery and it is work getting your routine down with the protein shakes and getting in all your fluids. Take as much time from work as they will allow. Your body has to heal and it does take time, but you will feel so much better. I wish I had done the gastric bypass instead of the lap band initially. I paid cash for my lap band and I certainly don't feel I got my money's worth with all the problems it created with my health and I lost very little weight. With the gastric bypass, the weight loss is slow and steady. It didn't start falling off right away and I thought, "Oh no", another failed weight loss program. But just stick with the eating plan and it will work. My highest weight was 280 and I have weighed 165 since Sept. 2016. Hope this helps, but if you have any additional questions, let me know.

Topic: RE: Revision to Bypass or Full DS ...Questions from Canada

I'm in the US so I can't even begin to answer the questions you have specific to your insurance. Down here though, GERD is usually enough to qualify for someone for revision to bypass.

OH has a forum for those in Canada that would probably be more helpful to you at the moment. This revision board doesn't get as much traffic but you might try the Canadian specific forum.

Good luck. It sounds miserable.

edited to add link to Canada Forum.

I've been researching Apollo Overstitch and found your post. I'm wondering how you are doing now that it's been a few months. Would you recommend the procedure? Any info is appreciated.

thanks!

Valapp, Hi and thank you for replying. I saw my doctor and after describing the symptoms of reflux, coughing/aspirating at night and terrible heart burn he will proceed on doing a RNY. I had so many questions for him but forgot them all even after writing them down..that being said really my questions are nothing anyone can answer since we are all different. Question for you: how long did you stay in the hospital? When were you allowed to return to work. If any complications that you might have encountered what were they? I have lots of people that are trying to change my mind to just removing the lap band and letting it be...however they've never been obese >200lbs and I know myself ...I will be going through the RNY.

Dr removed all fluid from the lap band and I felt an immediate relief. However I feel that were the band is placed it has to be raw....because the horrible heartburn in the opening of the stomach has been horrible..but thankfully the tums works!

Topic: Revision to Bypass or Full DS ...Questions from Canada

Hi there,

I'm from Manitoba. I was sleeved 7 yrs ago Feb 2012 in Richmond, BC but due to my unresolved medical issues including Sleep Apnea, Hypertension. Oh, not to mention that Post surgery I developed GERD and was recently diagnosed with Severe Osteoarthritis of the spine. Now today with my weight at 275lbs, I am still not at goal(135lbs to go) and in far too great pain & discomfort. I need to consider a revision to Bypass or a Conversion to the full DS surgery.

Don't get me wrong, my sleeve has been the best to me since I was 429 lbs on my surgery day with my highest over 500lbs and barely living(my lowest at 247lbs). Yes, I have regained (30lbs so far). With my spine in such condition, I simply cannot afford the lbs. I've had enough of the excruciating pain and all the medication that I'm on. I have tried "Rebooting my Sleeve" to no avail. So it's just not enough anymore..

Out of Country/Self-Paying is out of the question.

What are my options? Is Revision even an option for me? Where do I start? Im not sure i qualify for the program in my home province since I had surgery Out-of-Province but I've heard Alberta has a Revision Program, is this true? I'm aware that Ontario does the DS Surgery but is that only for ON residents? If not, who do I contact?

Please Help me.

Thank you in advance

Topic: RE: Gastric Bypass Revision with complication - AFIB

I'm unclear about exactly what you're referring to when you say you had a revision. Did you revise to RnY? Or did you revise to an E-Rny? Or from a sleeve? Or lapband?

As far as your Afib goes though, I can't imagine any bariatric surgery that causes atrial fibrillation. Maybe it uncovered something pre-existing?

Or even more likely, it may be related to dumping syndrome as Hala pointed out. What have you been eating? Do you log your food and maybe have a record of what you were eating that day? Because that would really help us here to give a better answer.

I'm not trying to minimize your Afib. I've got heart problems myself and my Mom recently went through over two straight weeks in Afib so believe me I know it's a scary thing. Just thinking out loud here because if it was a result of dumping syndrome (and rapid heartbeat IS a symptom of dumping) you'll be able to actually control your episodes.

Have you had other episodes in the past four months?
