Sleeve to bypass with Medicare

has anyone been approved through Medicare for sleeve to RNY?i originally lost 136lbs, but now have about 70 to lose to reach my goal. I meet the guidelines except being obese for the last 5 years. I started regaining about 3 years ago. Any input is greatly appreciated!
