HAD RNY REVISION DUE TO MALNUTRITION

I havent found a lot of information on this when I researched the past 6 months. Just wanted to share this. I Had  Rny revision last week due to malnutrition.  I was so malnourished by the day of surgery, I honestly did not think Id make it. Im not sure what all he did , I was in surgery for about 3.5 hours. When I got out they did start a central line and started me on TPN W/Lipids (Total Nutrition through IV.) I also came home on the same. Im running a 2000cc bag of what looks like milk through my central line 12 hours every night. Hopefully this will give me a good head start. My dx, I noticed on my paper work were Malnutrition and failure to thrive, among other things. That was pretty scary to me. I have a follow up apt today and will have Dr. Spell out what all was done again. I was just not very alert to remember. 


Ive read the studies on the ".gov" sites and I know bariatric surgery has been a huge success. It has saved lives and eliminated many diseases. People have complications and die from having their tonsils out. I also know the sleeve has a lot less complications than the Rouxen-y. For those of you who have had this surgery or are thinking about it, JUST BE CAREFUL. Even if you had it and have been doing great. I was 12 years out.  Had RNY 2004 - weight - 250#. Got down to 140#'s. Had no complications the first 8 years. I had some obstructions and bleeds after that but maintained a final weight of 175 ( size 12) for many years. I was very comfortable there. I started getting sick, loosing weight about a year & 1/2 ago. It just never stopped. Theres no guarantee all of my symptoms were related to this, but Im praying they were. Time will tell. I weighed 126# the day of surgery. Im skin and bones, have had no activity, burning no calories over the past months.


They are building a Spectrum clinic somewhere along the lakeshore that will be doing bariatric surgeries. It will be a common procedure one day if it isnt already. I know there was a time Id have given anything to be thin. Now I just pray I can get out of bed and have somewhat of a life. Ive talked to a lot of people who have had problems. I appreciate all of the feedback. Everyone has their own opinion. Feel free to share this with anyone. Ill gladly help anyone having problems. You definitely have to be your own advocate and have some knowledge base to get medical help anywhere now a days. I also didnt know this but Dr. Gluck can send you over to his floor at Mercy for hydration and /or Nutrition via IV. You dont have to go through ER. I did that once but wish I had known that was an option sooner.

From year 11-12 I also started losing weight quickly and had not changed anything. I remember thinking omg what if it doesn't stop. I had people asking me if I was sick! But then at year 13 it stopped and I don't know why.This is some serious surgery and needs to be really researched before doing it. I would do it all over again but I can see this is for life !! I pray you get better

Kathy, thanks for responding. Its funny (not really) but when I first started loosing weight, about a year ago, Everyone else noticed the weight loss before I did. My boss called me in and asked me what was going on. He said it wasnt safe to loose weight as fast as I was. I was wearing uniform scrubs and really hadnt noticed. Hopefully this fixes the problem.

The blogs seem to indicate that such complications are rare? What state are you I ?  How are you now?   Get well soon .

I am in Michigan. I'm 2 weeks post op now. Feeling better. On liquids only and still getting most of my nutrition through IV. I talked to dietician, they had calories at 1100 to maintain 128#'s. Got it increased to 1600. I haven't felt hungry since day of surgery. I felt sick "starving" feeling in pit of my stomach for awhile there. 

Dr did say I had a ton scar tissue from previous surgeries in pelvic area, took an hour just remove that. But that wouldn't cause pain after 1-2 bites/ drinks. I also had scar tissue around top, opening pouch. He put stomach back together, removed areas on top stomach/pouch. Removed some bowel. Then converted to a modified Sleeve. It is smaller and shorter than normal sleeve. 

I've spent a lot of time researching online. I found 1 news article of someone who lost weight very similar to me. They couldn't find problem. As she got worst, couldn't walk, ended up in ER, died days later. I can add link to that article.  But that is the ONLY place I've found anyone with extreme malnutrition. So that is really good news. 

I've read very low B1 and B3 ((Thiamine & Biacin I think)can cause a lot similar symptoms, neurological problems and eventually death. That was interesting treated by just taking oral supplements.  Vitamins are SO IMPORTANT

Hello! Hope this finds you well. I find myself sitting here 6 years out from RNY today weighing 103 pounds. I've been battling too much weight loss for close to 2 years already. My surgeon and GI have ruled everything out. Now my surgeon days I need a revision. He's not going to touch my pouch, just shorten the route the food goes so it won't have the chance to malabsorp as much, leaving my body to get more of the calories and nutrients. I know something has to be done but it scares me to death the thought of gaining a lot of weight. I want to get where I need to be and maintain. He also said I will have to have a feeanymore ding tube for about a month. That does not make me happy. I have fought him over this surgery but he says it's my last choice and if I lose anymore weight I am putting my life on the line. So much to think about, and nothing to read about... Everything is about failed RNY with weight gain. Any thoughts? Ever hear of this surgery? I trust my surgeon... I have had countless surgeries with him so I know he is excellent. Didn't figure 6 years out I'd be dealing with these issues!

Hi, Just read your post. I too am afraid I'm not going to make it if I continue to loose. I'm down to 109#'s. I did anticipate loosing more post op before I started to gain. It doesn't hurt when I eat now but I get really full and can't eat much

Are you able to eat a lot? I'm wondering if you can actually eat a decent amount of food and it doesn't absorb or if you can't eat much and if so why. I do have some thoughts depending on your answers 

Are the doctors you are seeing specialists? Have you had a second opinion? I'm thinking about going to a specialistat U of M. 

Most food is absorbed in the intestine,  not sure how removing some of it will help. I also wonder why a feedING tube post op. Unless they plan to put the tip in a particular spot in the intestine as opposed to the stomach and bypassing areas. If I had to have a feeding tube I would beg them to put it in my stomach, cut small slit and have it go in that way. I know, since it's temporary, they usually put it in through the nose. I COULD NOT HANDLE THAT, EVEN FOR A FEW MONTHS. 

I searched everywhere looking for others who have lost too much. I know how frustrating that is. 

How are you feeling mentally & physicaly? I know before my surgery I could barely walk aND I couldn't even remember which tooth brush was mine. Now, even though I've lost more weight, I'm 100 times better. 

What about your labs? How is your protien, albumon, pre Albumin levels.

I am 5 years post Op and have gone down to 93 lbs recently. I have had chronic diarrhea since day 1. I am always in pain and very nauseous. I take pain meds and zofran every day or I would not be able to eat. I am currently hooked up to tpn for nutrition. I have gone to the Mayo Clinic here in Minnesota and they said they do not see anything wrong inside of me and cannot do anything for me. I cannot live like this. I sleep constantly and have absolutely no energy or stamina. It is like I am just waiting to die. I have no quality of life. I am considering asking for a revision just to see if that helps. I am hoping that someone out their in the bariatric world has any ideas? I am only 53 years old and am currently raising my 9 year old grandson. I need help!!! I am very scared to have a revision as I am in such poor health. Thanks in advance for your help!

vkhill22, hi , You described EXACTLY how I feel 24/7!!!!! Since my revision in July 2007, I basically have been living in a recliner. I cant even lay on my bed because I get such bad pain in my back and cant pull myself back up. Ive been diagnosed with hypersomnia because I cant keep myself awake forcing me to take Adderall 30 mg 3x`s a day and 200mg Provigual 2x a day just to try and keep me awake during the day. I did NOT have Hypersomnia before having RYN Surgery . The reason I had to have a revision was because the 1st surgeon did not stitch my pouch closed completely and with all the acid reflux I get, the acid was seeping through and basically burning holes and it was 95% bleeding Ulcers  6 months after my original surgery. I couldnt eat or drink ANYTHING. The 1st surgeon kept brushing me off telling me I was fine . I was told from the gastro dr that discovered the ulcers I had to find a surgeon ASAP. No dr wanted to take on another dr`s work. Luckily after calling several surgeons I explained what was going on with me and had the gastro dr fax my endoscopy results to a surgeon that actually took the time to listen and he had me come in the very next day. Within 5 days he had me scheduled for emergency surgery. He didnt even accept my medicaid! I was in surgery 8 hours that day and was told I would have died if I waited any longer. On my follow up app after that life saving surgery , when I went in his office the entire staff stood and clapped,,, I guess no one thought id make it! BUT.....Now, 10 years later, I have NO quality of life. I have to have an aid 6 days a week. My only outings are to Dr appointments. I have NO energy to do or go anywhere, in the last 2 years 75% of my once beautiful white teeth just fell out, Ive broken 3 bones very easily. Im VERY deficient in vitamin D, I do not obsorb anything, I actually (and finally!) found a dr who gave me 6 iron infusions but I got no energy from that like I was praying I would :(, Ive had dumping since day 1. Im constantly dehydrated and sometimes cant even get labwork done. I just turned 49 and look at least 60-65. My skin, hair and nails are terrible. Ive been to so many dr`s and no one does anything to help me. I hope I can even remember (because I have a very hard time remembering anything now a days) to find this site again to see if anyone replied anything helpfull for me. I told my 2nd surgeon one day after he attempted putting a feeding tube into my stomach, " ya know, sometimes Im upset you saved my life" he asked me why? I said " because I have NO quality of life"  Its not to fun sitting here day after day night after night, taking meds to stay awake , bored, depressed, sad and alone and always hungry since my food goes right through me. And the government gives me a huge $76 a month to buy food! Who can afford meat/protein on that? So sorry im rambling. Im going to try and sleep. Hopefully someone can tell me something helpful.
