Maryland?!

Anyone had a revision done in Maryland? I'm having issues talking to my insurance company. They (United Healthcare) say I have to do one full year of the documented weight loss attempts. But my surgeons office said that can't be true because the state of Maryland has a mandate saying it's only to be 6 months. Has anyone ever been told this? What was the outcome? 

I am having a revision done in Annapolis. My insurance required that it was medically necessary.
