Whats considered medically neccessary for RNY revision to DS?

If the anastomosis  too big 25 cm is that rational for RNY to DS revision? Is it considered a mechanical failure or some type of failure?

the definition of what medical necessity would be in your specific subscriber agreement and you would be surprised how different they can be between policies
