RNY Revision for Marginal ulcers

Hi! I am meeting with my bariatric surgeon tomorrow to schedule my RNY revision, total gastrectomy and J-Tube placement. I have been fighting marginal ulcers for 18 months and I have failed high dose PPI and round the clock carafate Q3hrs. I have had 5 GI bleeds(2massive). I have a multitude of underlying health concerns anemia, malnutrition(obviously), sphincter of oddi dysfunction, chronic abdominal pain, nausea & vomiting, chronic R Flank pain, B-12 deficiency, POTS, mitral/tricuspid valve regurge, and anxiety & depression(I have no idea why I would be depressed...lol) to name just a few. I relocated from Chicago to Phoenix in July of 2012 & had to start basically all over again. I have 2 gastroentrologists, an Interventional GI, bariatric surgeon, hem/onc, pain specialist and my Pcp. I am just wondering if anyone could give me any information about revision. Also, I(fortunately) have never had an issue with weight loss. I went from 258-92-120....I am stable at about 120. I am worried about being unable to maintain my weight and not slip back into the 90's. Any information is greatly appreciated.

They're going to completely remove your stomach??? Have you discussed possibly being revised to a Sleeve instead?

Yes...my bariatric surgeon is going to do a total gastrectomy. I have a small fistula that was seen on CT's but not on my EGD's. He feels that acid from my remnant oz traveling up through my intestine and through the fistula. Also, because my pouch is so tiny after they revise the anastomosis it will be esophagus connected to intestine. I am really afraid this time. Although I haven't worked since 3/2009 due to complications I am a nurse and a paramedic....so I naturally make the worst patient on the planet. No one ever suggested sleeve. It's possibly because prior to WLS I had struggled with peptic/esophageal ulcers???

I had RNY and ended up with a fistula in 2005.  Three weeks ago I had a revision from the RNY to a DS. My stomach is so much happier. I never did lose much weight so for me I am losing again but I would certainly talk to them about the sleeve. It would at least restore your normal stomach. You obviously don't need restriction or malabsorption but would leave you with something rather than nothing. And maybe it would help with any weight regain. I certainly am not an expert but I would check into it.
