DENIED...Desperately Need Suggestions/Advice - Appeal Letter

I have been denied by BC/BS for my revision surgery and I desperately need HELP!  I have to write an appeal letter, but have no clue where or how to begin!  If anyone have suggestions or can give me advice on the appeal process I would REALLY appreciate it!  I really don't know where to begin!

First you need to find out why you were denied, than you can know what to address in your letter, also your surgeon should be able to help too.
