Speaking about Iowa - revision with Wellmark? BCBS of Iowa

Has anyone here gone thru a revision with Wellmark (BCBS of Iowa)? I've called several times about their revision policy but was told revisions are handled on a case by case basis.

Yes, I've pulled their policy and their revision portion states that if a patient does not maintain the lifestyle required by their surgery a revision is not medically necessary. I have multiple complications from my band and going thru the process for a revision/removal of the band.

Even when talking to their reps everything is on a case by case basis and no criteria is listed.

I do have the once a lifetime surgery line. Right now I'm in the stance of I don't care about a revision - I want the band out due to GERD, can't keep food, liquids down and etc.
