Letter to insurance for revision

I had the lap band procedure but it is not working for me and my doctor recommended gastric bypass for me but I have to write a letter to my insurance explaining everything I have done since having the lap band surgery but that it is not working for me. I am not sure how to write the letter, does anyone have any suggestions or example letter to give me an idea. I have followed my doctors instructions before and sincemy surgery but it's not working and he has recommend the gastric bypass and is sending a letter to my insurance but he says I also have to write a letter to them. I sure can use some help or suggetions.

You are going to need to change LOTS of wording since they are both different procedures.... but it is a good general outline, at least.................... A place to start.

Are you having lapband complications?  I had to appeal and won and am having gastric bypass on August 1st.

I have letter but i would need to copy it and put on my blog or send to you or whatever.

Hi, No I havent had any complications, I cannot eat any type of breads / dough products cause they will get stuck and then I vomit so I know not to eat them but other than that I have not had any problems. You can send me a copy to my email [email protected]. Any advice would be great.
