insurance problems for revision

I have had lapband for almost two years and I,m not having great success. I am thinking about a revision to rny. My doctors office does them, nut they said my insurance is not paying them, and the only hospital that will take my insurance has told them not to schedule more surgerys because of it, even if I can get them to approve it. Any suggestions. I have a hmo through medical.
