Revision? keep your old surgery approval letter!!
It's my fault for thinking my chart was safe in Dr's possession!!!
My wls date has changed, because I have to locate the paper.
The original dr moved after 4 years and All the charts had to stay with the hospital he was with. So they scan all records, buy guess what since it is a medical facility, they did not scan Insurance from the Dr. only medical records and ect related to hospital.
I have called the original Ins but it will take another 2 to 3 weeks to get.
Sooo lesson learned, I now have ALL copies sent to me and DR.
psyc, nut procedures, etc. And will get copies of office visits too when I go next.
The new ins needed original approval letter to meet the criteria of being covered on FIRST wls in orfer to cover the revision......
Good thing I don't stress easy!!!
