HCA revision approved?

Has anyone had theirs approved? Dr office called and said hca does not approve revisons, on

aetna ppo with HCA .......says will only cover one in a lifetime, doesn't matter who paid for it ????

Has anyone else stumbled on this before???  United healthcare did my original band, now

going for revision due to it leaking, needs to be replaced, also having problems with choking etc,
