i have tried numerous times to contact dr husted with no results. i even left a voice mail with his office and no call back. has anyone else had this experience?
Try calling Commonwealth Bariatric Center @ 1-800-426-4285. It is easier calling them than getting a hold of his office. That's who I called and got a patient packet. Their web site is www.commonwealthbariatriccenter.com - I hope this helps.

Thank you everyone. I got a call today. Got my paper work off his site and will get started. His staff gave me a diagnosis code to call my insurance company to see if it is approved or not. My insurance company didn't have that code. Here we go, but at least I am talking to someone.
