Amerihealth for revisions?

Has anybody used Amerihealth for revisions?
They are not helpful.
I really, really need a revision. It's not excluded in my policy booklet, but I did get them to send me an internal medical policy bulletin states that they do not cover “dilation or stabilization of the gastric pouch as a natural and ordinary occurrence in the aftermath of the first bariatric surgery."
I don't think that "natural and ordinary" changes have been documented in the literature. Or have they. It looks like Rose and Stomaphyx, which seem not to work, have been performed on people with stomas greater than 2cm. Mine is 4cm.
I am looking for any insight on this company.
