Revision Coverage

Has anyone had successful approval s with UHC on revision . I have UHC-EPO.

my surgeons office just submitted me yesterday to insurance and I have UHC PPO.

I wanting to have a revision from Lap band to RNY.

I have UHC EPO with the state of arizona and I was approved for a revision.  They have very strict guidelines that need to be proven.

I had a revision in Feb. of this year. I have United health choice plus. I had a revision from rny to an extended rny. I was suppose to get a ds but too many complications once they got in to surgery.
