Nebraska Federal Plan??? For DS Revision by Dr. Anthone in Omaha???
Well I started out with wanting a revision from the lap band to RNY, then to VSG, now to DS.
What are the requirements this year 2009 for a revision to DS with Nebraska Federal Plan? Please let me know. Thanks.
