bcbs of ks does it cover revision???

I have regained about 30 pounds in the last three months no clue really as to why except i am able to eat more before getting full... I am 5 years out and have stayed the same weight for four years now ame regaining all of  a sudden.  I am considering having the revision done but need insurance to pay because there is nooo way i can self pay unless they would take payments.  I have BCBS of Kansas... Any help would greatly be helpfull.. Thank you
