BC/BS PPO in Utah

I am needing to get a revision from RNY to DS and am concerned that my insurance won't cover it. I'm going to go to Dr. Husted and I know that I will need to have a series of tests done first but I am just curious as to what they might be looking for in approving revisions.

I had BC/BS and was able to go to the site in my state and obtain the information.  All the best make sure you check out whether you need the extended version or not.

I tried finding information about it but couldn't.  Maybe I was looking in the wrong place?  What do you mean extended version?

BC/BC has a site and that's where I downloaded the info.  Ask the rep from BC how to download that info they were good in answering my questions.

I had the RNY in 2003 in 2008 it was extended to include more intestine bypassed.
