Sample letters to insurance company?

I am looking for some sample letters to view to help give me and idea of what all needs to be included to have revision surgery. Anyone got a clue where to find them?

If you click on the forums at the top of the page you will see where you can click on more and then on the page that comes up scan down to insurance forum.  There is some good info there and a letter that I revised a little to make an appeal to the insurance company.  Good luck.  More power to revisions!
