Revision: Does GHI and Empire B/c B/S require a 6 month diet history

I am meeting with my surgeon today to discuss Stomaphyx.  When I had my RNY in 2003 I had a huge problem with GHI approving my surgery and Empire denying it b/c it was not medically necessary.  I found out yesterday that GHI will cover a revision, but I am not sure about Empire.  I don't know if they are going to make me diet for 6 months.   Has anyone tried getting a revision with these two insurances.

 I can't really help but I am meeting with the doctor for the same procedure and the office told me it was next to impossible to get the procedure approved  It is pretty much the same as the lapband prior to 2005 since it was a fairly new FDA approved procedure.  Considered experimental.
