how long does it take to get ok for surgery from insurance

That is not an easy question to answer.  It depends on so many things, including on the insurance company and how long they have to approve.  for example if a company has 21 days to say yes or no, some will take the entire 21 days, others will get back to the doctors office in a week or so.  Have you checked with the surgeons office, they should know on average how often that particular company takes. my revision approval took 7 working days.
