Insurance questions regrading revision surgery?

Hello, everyone. I had the stomach stappling back in 92' I did fine the first couple of years then the weight started coming back on. After countless diets i am now at my heavist weight,315. I have been going through the 6 mths dr. suppervised diet, hoping to have the revision surgery done. My insurance is BC/BS of Alabama,I have been told they are hard to get the revision surgery approved. I was wondering if anyone else had this insurance and want find of luck did you have in getting the surgery approved?
