Did anyone go thru Dr. Steven M. Clark in Indiana?

I had the realize band on 12/10/08.  I need to know does anyone have Dr. Clark's 12 digit Surgeon's Practice Id no?  I want to register on the Realize Band website and I need this number..thanks....

I went to him, but I don't have the number, I hardly ever use the website, I know I should though...call his office, they will give it to you, that is what i did at first.
How are you doing with your surgery?  I had mine on July 25 2008 and I have lost 59 pounds so far.  I honestly have not been exercising lately though, i know I need to.  I need to go in for another fill, it will be my 4th.  I have 5.5cc's in  now.  Good luck with your weightloss!!

Mine is doing ok, but I can't wait to go to my first dr. visit on Jan. 5th.  I am hoping to get my first fill.  I have lost around 20lbs hooray on my scale, but when I went to the dr. the other day it showed that I only lost 7lbs, I was really bummed.  I wi**** would come off quicker...
