Realize my Success problems

Ok, so I got the little card thing from my surgeon which says that I was banded, it even has a patient card ID, but when I try to sign up on the realize website they say that you need your Surgeon's Practice ID- what is this? Is there anywhere that I could find this out besides in my doctors office? Does the ID change when he leaves a hospital for a new one or does he always have the same one?

I had to call my surgeons office and get it from them. They always have the same one I believe.

your surgeon's office has an id # and you get the patient id card...call them and ask them for the office id you need both to register
