FMLA forms

I got a call from my HR dept today stating they received my FMLA paperwork but my doctor only put office visits on there. Nothing about incapacity due to vomiting.  They asked me to have their office resubmit the forms.  I called the office, and they said they didn't know what I was referring to about time off for "incapacity".  Did any one else have issues getting approved FMLA for vomiting?

Hey there,
It may vary from state to state, but here in Caligornia, all I had to do was get a doctor's note that excused me from work with an effective date. So it said "So and So is excused from work due to medical neccesity as of 1/1/2011 until further notice." My emplyer really had no right to ask about why I was going out. Hope that helps, i know there are lots of ladies on here who are more knowledgable on the subject.  Good luck!

you need to get your doctor to do a form for both intermitten which would be for the vomiting/excessive breaks etc.  the other would be for extended fmla for when the baby is born.

 I have STD disability to handle my time off for delivery.  I only use the FMLA when I have an appointment that conflicts with another employee's time off request or when I'm not able to work due to other issues.  I've been handling my nausea well now that I have pills, but I have about 8 hours total in the past couple of weeks that if I don't get FMLA for , they go down as unscheduled and count against me.  

When I was severely ill in 2009, I came close to maxing out FMLA.  I maxed it out in 2010 when I had my bypass, but I still got paid because I was on STD.  They run FMLA and STD concurrent to one another, but if you max out one you still have what's left of the other.

I'm getting new forms and I'm going on a day where I have nothing else to do but get this done.  I don't want to take a 8hr unscheduled hit.  We only get 24hrs a year.  I used one this year for a different illness.

I agree that you need to get intermittent FMLA which will allow for things to include your visits, days off due to nausea/vomiting, fatigue, etc
Only other thing to keep in mind is that depending on how much FMLA your company gives and when it renews that depletes from your FMLA balance/time you have for after baby is born. I wanted to take 12 weeks but only had 6 to take because I had been on bed rest and taken other time off for illness related to pregnancy.
