Protein in Urine ....PIH

My first two pregnancies, both boys, I had PIH. Both were pre-WLS. I had my first son at 37 weeks and he weighed 4lbs and my 2nd was born at 34 weeks and weighed 3lbs. I started PIH at around 28 weeks with both and they both stopped growing/IUGR.
This time, I am seeing a different OB at a different duty station on post and the protocol is different. Even before WLS, I had no Blood pressure problems outside of pregnancy, only when I hit 28 weeks on... it slowly started creeping up. Well, I went in for my 26 week apt today and my bp, which had been around 110/60, was 137/ 89 (slightly higher than it had been, but slowly creeping as I approach 28 weeks, or could have been ANYTHING). I have the signs approaching of PIH, seeing spots, swelling of the legs, but no weight gain I actually lost 5lbs. Well, they did a 24hr urine collection on me 6 weeks ago and just notified me that my levels were 288. Anything over "0" is considered protein in the urine. So I asked my OB what he thought or if he wanted me to redo my 24 hr protein (with my 2nd son I had to do them 2x's a week to check my levels and they said they were in the 100's and still considered PIH). He said he was not ordering another and he would not react until I hit 300+. Uggghhhh.... I have all the symptoms coming on and I am almost 28 weeks. I was just wondering if anyone else had any problems with protein in the urine and if it meant something other than PIH in WLS patients. I read something about Keytones in the urine and I don't know if that has anything to do with it or our levels could be off from WLS.

I have had ketones quite often in my pregnancy.  It could be a hydration problem, or my bariatric surgeon said it might be just a remainder of the pre-diabetes I had before my DS.  I actually tested positive for protein several times on the dip test (BP is fine, was bad in pre-op pregnancy.)  But my MWs had me start doing clean catches and then no protein.

keytones just a fancy word of being dehydrated. I have had them in all my urine tests that I have had. My dr witll then order blood work to see where my potassium levels are. If they are extrememly low then i get  a bag of saline/potssium.

AS for the proteing, its a hit or miss with the protein in the "dips". IF you are having symptoms then I would INFORM your DR to start to pay attention and you WANT to do a 24hr test...just to start to get a baseline.

Thank you ladies! I am calling the OB department today to complain. I am just not happy with my office visit. I did a lot of research last night too. I might just go to labor and delivery and get checked out to see what they say.

Let us know how you are feeling. My levels are always high too but we kind of go by a day by day kind of method. Guess each doc is different but who knows. Just be proavtive and do what makes you feel better cause you are the momma and its you who has to feel comfortable. Keep us updated.
