Need some guidance/advice... those of you with PCOS before surgery

I am 15 months post op and have lost 130 lbs... I need some guidance/advice anything you can share...
Let me tell you what is going on... I used Nuvaring for the year during post op, stopped Nuvaring in December. My periods have been pretty regular (which were not before surgery). We are trying to become pregnant... I have been doing the ovulation test and past 2 months have been positive, this month didn't have a positive test. This week I had a terrible pain on my left side (ovary) so bad that I had to take a Percocet Monday night. I did have ovulation pain about a week and a half ago or so but still ovulation test was negative.
I called my ob/gyn Monday and talked to nurse because Monday I had terrible pain and I had very ;light spotting. Nurse told me to take pregnancy test but it was negative. So, dr wanted to see me today. She examined me and said it might be a cyst but wanted to go ahead and do an ultrasound. At ultrasound tech had trouble seeing my ovaries but didn't see a cyst.
Dr. talked to me and said that it seems that cyst might have ruptured, that lining looks like I will be getting a period in 1 or 2 weeks and if I dont' to call her. I had tears in my eyes because I thought that my periods were going to be regular now.
She said that sometimes they are sometimes they are not.. and if they are not, then she'll have to help me with it.
I am just feeling very frustrated, disappointed that now my body is just changing its mind. I really want to have a family... Dr. not worried about it.. she just told me we'll have to wait and see what happens and if I dont get a period she might need to help me.
I am just feeling very emotional today.......I need to get out of this funky mood.. the pain is less now much better than Monday and yesterday...
Have any of you gone through this? Any advice? Were you able to conceive? How do I know when to try if I don't know when I am ovulating??? I am just feeling down today.......

I was dx with PCOS when I was 16 and was on metaformin most of the time except after surgery but then afterwhile i went back on it.... we didnt use protection for two years and i did fertility friend and opk's got pregnant with baby 1 and lost him then started trying in dec 07 and got pregnant in may 08 but m/c very early...then I started on metaformin again and did clomid i used opks and had no luck i would get a postive but didnt ovulate till days later so my timing was off then i started using a clearblue easy monitor in june/july 08 after 2 cycles using it I got pregnant...due to pcos I was getting a surge but having a delayed "o" so with clear blue easy monitor I would get the high for awhile then get the egg symbol and would "o" within 24-36 hrs and was also temping....also if you r stressed maybe have a glass of wine or something to relax...the night she was concieved I had a few choco martinis and was so much more relaxed.
Are you on any meds?

I had PCOS before surgery, and still have it now. I am almost 7 years post op. I never had regular periods without help. Before we tried to concieve I used the pill. I truly think having been on Depo Provera at such a young age (16-19) also had some impact. Nonethless, I tried Clomid first and when that did not work was given other options through a specialist. I was able to conceive my son without needeing IUI Or IVF, only assisted with medications. He is now 2.5 and we are trying for #2 as my periods still have issues. Currently I take Metformin and then provera to start my cycle as needed, if this does not work we will move on to more aggressive meds/options as we did before. There is hope. Hang in there.
