Very OT: Termination Parental Rights

This is VERY OT, but I need some help quickly. I know there were a few people on here that had terminated parental rights on their ex and I need help. I have court tomorrow ( through DHS) and I was told to get an Agreed Order of Termination of Parental Rights to have my ex sign tomorrow, if he shows. We can both sign and have it notarized then I could file it. Well, I went to uslegalforms.com and got the agreed order, but it does not have a place for us both to sign. Does anyone have one that I could reference in getting the form set up properly?? Or does anyone else have any other ideas?? This is very short notice and I've tried 3 attorneys to help me and they want a big retainer upfront before they'll help. Please someone???!

I dont know if this will be an accurate answer but I think maybe your able to go down to the court house and pay for a copy of what you are needing. I know here in San Diego you would be able to do that. Good luck I hope you find out what you need to know.

Sorry I didnt see this sooner, I'm a social worker with CPS and have just the form you needed. Oh well, how'd it go anyway?

He showed up yesterday with his new wife in tow. I was surprised to see him actually show up. He gave the Judge his pitiful story of being homeless after being fired from his job. He said he got laid off... ha! Whatever. Anyway, he told the Judge he wanted to terminate his rights, so Judge gave him a continuance so he could get it done. So, now the new court date is for August. He is almost $9,000 behind... and the ex THINKS I'm just going to forgive all that.
