Baby Shower
Thankfully, my office is throwing me a shower...so that helps eliminate the cowork confusion. However, I know what you mean about the thin line between friend/acquaintance and being a "gift grabber". I am still debating on a few folks in that category.
Regarding out of state family, I say that it is always safer to invite them. That way they don't feel left out and it they want, they can send a gift.

I always invite out of state relatives to events like my DDs birthday parties and they'll be on my shower invite list as well.
I won't be inviting work acquaintances to my main shower but everyone usually gets a shower at work and anyone who likes me enough will come to that. I work pretty far from home so I doubt anyone would drive over an hour to come to my shower when they're having one at the office. If it weren't for that, though... there are some that I would invite to the main one. Probably a few that I wouldn't normally invite just because I wouldn't want their feelings to be hurt.
I think it only looks bad when you start inviting friends of friends that you've only met once or twice... I was invited to one of these and really got the feeling that she was just looking for more gifts. I'd only talked to the girl maybe three times over the course of 2 years.
