What does this mean??

It code really doesn't have anything to do with being able to get pregnant, but with the "outcome" of what happens to the pregnancy.  (Believe me there are codes for every kind of supervision of pregnancy.) 

From ICD-9:
The catergory V23.4 (Preganncy with poor obstetretic history) has two subcatergories:

V23.41- Pregnancy with history of pre-term labor

V23.49- Pregnancy with other poor obstetric history
