WLS-PCOS - How to get the Ins to Pay - And get started. Need Help!

Hi, I am new to this board and was wondering if anyone could help me. I have a BMI of 36.3, 5'6" 225# (or there abouts) and have known I have PCOS for about 10 years. Never knew any of the side affects of it til recently though. Except for infertility. I have HBP from being Pre-Eclamptic with my daughter and severely anemic (2 years ago). I've been researching everything about PCOS now and have found that I have a number of the symptoms: -Overweight (especially around the abdomen) I've tried ever diet and can't seem to lose weight. If I do, I gain it back and then some. Nothing seems to help. And the heavier I get the more depressed I get. -High Blood Pressure (from the Pres-Eclampsia, which I found out PCOS women are more likely to have during pregnancy - and a lot of women don't ever get rid of it, especially if you have a family history of HBP) -Excessive Hair (Face, abdomen, toes) -Irregular Menstrual Cycles -Acne -Depression -Skin Tags -Darkening of the skin -Pelvic Pain -Cysts on my ovaries - that burst and are comparable to labor pains Which my regular PCP doesn't really want to do anything about it. I'm on BP medication and Depression meds. I was on Ortho-TriCyclen to control the acne but since my DH had a vasectomy I don't need them for BC so I quit taking them. I have Coventry HMO insurance and I have already called and they do cover WLS but with sufficient proof of necessity. I would like to get the Lap Band surgery and plan on seeing an Endocrinologist here soon, probably after Christmas. But before going in I'd like to know what to ask or need to do before going so I am prepared. I would like to go in with the intention of getting the surgery and if anyone has information that would help gain an approval, I would greatly appreciate knowing how to write this stuff up. What to write, history, etc. Sorry this is so long but I tend to ramble when I type!
