Hi and welcome to the board!!  With diet and exercise and Metformin I was able to lose 50 lbs. within a few short months.  I maintained this weight for just over a year, and the weight started creeping back on again...ok...not really creeping, but packing on quickly again.  Once I regained 35 of those lost 50 lbs, I decided I needed a permanent solution, and chose wls.  Because I'd been seeing my doc for my PCOS and I followed her diet suggestions, she had documented my weight progress, etc.  I didn't need to do the 6 months diet...I'd already been under her care for 2 years.  My insurance counted this time span.  Insurance companies vary on what they require, so I'd check with yours.
