FMLA and veterans of inner thigh lift and LBL

I am seeking the advice of all veterans who were approved for FMLA to have plastic surgery. 

What should I definitely write on the FMLA paperwork to insure that I am approved for paid time off (FMLA)?

Also, any veterans of the inner thigh lift and LBL at the same time - -  Is it possible to do this surgery with NO after care assistance (solo after leaving the hospital)?

Insurances normally require that there is some health issue that would be helped with the surgical reconstruction. This is usually something like having difficulty walking because of the rubbing of the thighs or skin/pubic area irritation.

Although paid time off (PTO) is related to FML (Family Medical Leave)...they typically mean different things.  At my job, you can apply for FML regardless of what type of medical procedure you're having and you'll get approved.  You can also apply for it if you're having a kid, adopt a kid, have a family member with medical issues, etc.  FML protects your job, especially if you exhaust your PTO benefits and still need to be off work.  

Once I'm approved for FML, then I can put it for PTO to cover the time I'm going to be gone.   Does that all make sense?   I just didn't want you to think that FML automatically entitles you to paid time off because it doesn't.   If you don't have enough vacation/PTO time to cover the entire time you're off, then you might have to apply for short-term disability.  Each employer has different rules on that too depending on their insurance carrier.
