Documenting prior to TT surgery?

Hi all, I was sleeved about 18 months ago and while I'm still working on the last maybe 20 pounds, I've been starting to think ahead to hopefully an Abdominoplasty and panniculectomy, plus possibly a breast reduction. My insurance, Aetna, requires 3 months of documentation for both the panniculectomy (dermatitis) and the breast reduction (pain) and I'm curious if I can just go to my primary care doctor for that documentation or if I need to be seeing specialists for each. 

Any thoughts or experiences?

I am hoping it is just the PCP as I have made and appointment to see her.  She should know If I need to see anyone else.

In our experience, either your primary care doctor or specialist is acceptable to the insurance companies.
