Revisions Under Local in PS's Office
So what revisions would a PS do under local anesthesia in his/her office? Additional arm skin after an arm lift, bat wings from skin along the arm pit? Dog ears from a breast lift? Extending the incision from a breast lift further around the back? How long after the original surgery do PS typically like to wait to do revisions...six months? Is it a better outcome to wait and have all revisions with the second and final round of PS? Is it more expensive to do that because you then pay for anesthesia and facility services?
Dr. Capella told me that it would be better to wait for my second round of plastics in the OR to excise the little patch of fat that he did not get during my TT rather than do it in his office. He said it was more sterile and just a better environment. He won't even think about revisions until the patient is 3-mos out.
My surgeon will not do a revision before 6 to 8 months out and then he prefers to take care of it with our next round of plastics in the hospital for better over all outcome. If dog ears were my only revision though, he could take care of that in his office after six months to insure the best outcome and repair.
I live in Israel and after a medical review specifically for plastics, I had TT in 2011 and BL/BR in 2012.
Both were done in the same hospital by the same surgeons . I have wonder healing issues (really an anomaly to the staff as I do not smoke or have diabetes).
During my BL/BR the problematic area from the TT was revised.
However I still needed medical approval by the Head Plastics in Israel on my HMO for the dog ears and "bra blubber". He deemed it to be only Cosmetic and denied me. I appealed yet lost and paid out of pocket.
In my case my PS at the hospital after my surgeries being discussed in M&M that I could not have any more procedures - reeavluated my situation and was decided I could fix dog ears and bra blubber BUT only in hospital Day Surgery with Local Anesthesia and Only One Side at a Time. All these conditions to avoid wound healing issues. Lefty was done a month ago and I did have a few glitches along the way but considering how I heal, the PS' are happy. Righty will be done 18 February 2014.
I paid about $1000 for Lefty and I imagine the price will be similar for Righty. I have already saved a quarter if the amount I need in shekels.
I must admit I was all worked up emotionally on how much it would hurt, and it did hurt, but not nearly as much as I had thought it would.
I hope my experience has helped you.

I will have an I'm office revision on Dec. 13 to remove a small roll of fat & skin from my left side. My PS waits until 6 mo. Post op. I had my TT on June 10th. If I were going to have a 2nd round of plastic surgery in the near future I would wait. I hope to have a braline back lift someday but it will be a while.
Thanks everyone! I saw my PS today for my 1-month follow-up. He was very pleased with how well I'm healing and how the incisions look. He said I could have the areas next to me breasts excised in his office in three to four months. I can change the simple revision appointment and opt for outpatient surgery when he can also do the other areas, extend to a bra line back lift and do my thighs. We'll see if the money fairy appears between now and then.
