Appeal help

Wondering if any of you have written a letter for an appeal to your insurance company for panni removal and would be willing to share it with me? Or have any key works I should use in mine. Thank you!

Hi Mell! I have been going around and around with my insurance company since September of last year about this. You need to know what the specific requirements are for your insurance to cover the panni. With mine it is 3 months documented care from a physician treating recurring skin issues such as rashes, boils and other irritations and the panniculus had to be a certain size. I submitted dr's notes from each visit, receipts from filling prescribed ointments and pills, a referral letter from my regular dr, color pictures of the pannus, and a letter from me detailing the difficulties associated with having this. Including, back pain, hygiene issues, sexual issues, depression, difficulty in moving and limited exercise opportunities. I really focused on how I had worked hard to be healthy and changed my life yet this is something I can't change and is reconstructive and necessary. I am going to send you a pm.
