Difference between an extended TT and a LBL

My PS wants to do an "extended tummy tuck" and the way he explained it was the incision would go past my hips and around my back a bit.   

I am a little confused about the difference between what he described and a LBL - and I'm reading conflicting things online.  Is a LBL a full 360* cut, as in all the way around and an extended TT is just anything beyond a standard TT (and how far beyond depends on the patient and surgeon)?  Or is there also a difference in how the skin is pulled/removed?

I know this is all "just semantics" but I want to better understand what people are talking about when they use these terms.  Maybe there is no straightforward consensus?

I had a LBL and yes, it is a 360 degree incision.  On the front, the skin is pulled up and down, and excess is removed, and the stomach muscles are stitched together.  On the rear, skin is pulled up on the thighs/buttocks and down from the top of the butt, removed and you are stitched back together.

I chose the full 360 because I had two quite large pockets of skin/fat on my upper buttocks that needed to go....my outer thighs also look much better.

I had the extended first - it went just past the hip bone on both sides.  I returned two years later for a LBL and it went all the way around.

What was the problem with the original TT that made you choose a full LBL rather than just going back for something like the Brazilian Butt lift?

The original TT was done after losing over 200lbs.  I had a lot of excess skin (19lbs) and chronic back pain.   He repaired a hernia and removed the skin.  I lost another 70lbs + and returned for my LBL with Butt Augmentation.

The LBL is the way to go if you can!

Thanks for your responses!  I do have some sagging skin on my butt, but the main problem is in the front and the hips.  I'm looking for improvement and not perfection... but, I do know that many say once you fix the front, everything looks even more saggy by contrast!   LOL!  I can imagine that is true.   I am hoping I'll be satisfied with "just" the extended tt, and it was what my PS recommended.

Have you consulted more than 1 PS?  It also depends on how much loose skin you have on your low back and butt.  I went with the LBL with the butt flaps for a rounder top on my upper backside.

I'll admit that I'm not the "model patient" in that I didn't go around and interview/consult with at least three surgeons.  I went with the surgeon recommended by my bariatric surgeon - he sends all his bariatric patients to this guy so he has a lot of experience in working with loose skin issues. He's board certified, never had any public complaints or legal issues, and his portolio of pictures looked good.   That was good enough for me. 

 

You know, I don't care too much about  my butt - mainly because I don't really look at it.    I can see, though that once my stomach looks nicer that I might think my thighs are too saggy and the shrinkles on my butt may bug me... but I don't think (think is the operative word here...my thinking isn't always right!) that it would bother me enough to warrant more surgery.

 

On a sort of side note  - I'm getting so excited and nervous!  My first round of plastics is coming on Valentines day!  (Arm lift and neck lipo)  I have my pre-surgical appointment for that surgery on this Friday!  Yeaaaa!!  So excited and nervous, I can barely focus on anything these days!

Oh, and I forgot to mention that I don't really have any loose skin on my lower back - but my butt does have some shrinkles on the bottom of it.    The bulk of my excess skin and stubborn fat is on the front - hanging down.  A pannus, I guess.

I had an extended TT as I thought my tummy, arms & breasts were the main thing that bothered me (I couldn't afford to do breasts/arms at that time) and I was fine with my legs & butt.  Apparenlty I just never really looked at them because my tummy looked so disgusting to me because once I was healed from the tummy tuck, I looked at myself in the mirror and my first thought was "wow, what the heck happened to my thighs - they're so wrinkly & droopy, were they like that before or did it happen overnight!?!" and immediately wished I'd done the LBL. 

Highest 323 / Surgery Day 289 / Current 165 - RNY 10-27-07, Hit Goal 08-18-08. Tummy tuck 10-28-09 - UW Plastic Surgery Residency Ctr, Breast/Arm Lift w/Dr. Sepehr Egrari in Bellevue, WA on 5-22-13!
