state insurance????
The biggest hurdle will be finding a PS who'll accept MCR/MCD. Most of them are reluctant to even take regular insurance, let alone a reduced payor like state insurance. The best thing that you can do is call around to the surgeons you want and talk to the front desk staff. They'll tell you up front if A: the surgeon accepts your insurance and B: if they've ever had any success getting them to pay. Good luck!

Couch potato to runner in 18 months!
I'm on medi-cal, the California state insurance (for poor people or disabled). I just got mine approved for panniculectomy but if the doc deems it necessary insurance has to pay for the full TT. I had to take it to court though and finally won.
