Need advice for appeal after denial for Tummy Tuck from Harvard Pilgrim

So, I attempted to get a portion of my lower body lift and medial thigh lift approved by Harvard Pilgrim only tro be denied.

PLEASE - anybody that has any ideas, please respond. My surgery in on 8/29.

You're probably going to have to provide more evidence that the extra skin is bothering. Or you're going to have to save money and pay out of pocket.

I have been denied for a panni twice now.  They finally told me I need 3 to 6 months of PCP visits for recurrent rashes with a dermatologists confirmation that rashes are not responding to treatment, proof of being more than a year out from surgery, proof of having a stable weight, and proof of over 100 lbs lost.  Oh, apparrently if I had ever gone septic and almost died they wouldn't have a problem covering it either, isn't that nice of them??  Anyway, I am on month 5 and will be seeing a dermatologist this month and next month and then hoping to have surgery in October.  Third times the charm right?  Also, October is my 2 year surgiversay so that works for me.
