Abdominoplasty requirements per Apollo Managed Care?

I am working on getting a panni approved through insurance and finally found out that what I sent in (been denied twice) does not meet the criteria through Apollo Managed Care.  They finally listed what they needed, so, in a few more months I will apply again witht he documentation they need.  However, I would really love to get a full abdominoplasty approved instead of just the panniculectomy.  Does anyone have any experience with Apollo Managed Care requirements for abdominoplasty?  I found it online, but can't access it without paying $675 for their manuals.
